Form 941 (Quarterly Report)
Filing Form 941 
To file Form 941, do the following:
- Log into eSmart Payroll
- Click 'Start a New Return'
- Click 'Form 941' to begin the online payroll form
eSmart Payroll allows for e-filing of current quarters and up to four quarters prior.
Note: Process requires a PIN (acquired through IRS) when e-filing.

Form 941 Information
Form 941 is the employer's quarterly payroll tax return. Every employer is required to withhold a certain amount from each paycheck for income tax purposes.
Use Form 941, Employer's Quarterly Federal Tax Return, to report the following:
- payroll taxes (withheld federal income tax and both employee and employer shares of social security and Medicare taxes) for the quarter;
- current quarter’s adjustments to social security and Medicare taxes for fractions of cents, sick pay, tips, and group-term life insurance; and
- prior quarters’ adjustments to payroll taxes.
Do not use Form 941 to report backup withholding or income tax withholding on nonpayroll payments.
Filing Deadline
All employers are required to file Form 941 before the end of the month following the end of the quarter. The dates are as follows:
